camp forms
Camp forms provide the Rec with all the information we need to ensure your children have a fun and safe summer! When registering your children for camp, we suggest you plan about 10-15/min per child. Please DO NOT do these forms on your phone. Last year we heard stories of the forms not saving and frustrated parents. PLEASE USE A COMPUTER!
FORM COMPLETION DEADLINE: May 1st 2024
If we don’t have your forms by May 1, you will forfeit your child’s spot in camp, with only a 50% refund. Again, Camp Forms ensure the safety of your child in camp and the some of the information that is provided allows us to plan accordingly with staffing, logistics, etc.

To register online you will need your online ID and password ready for online enrollment!!
To make the process easier please go online ahead of time and test to make sure that you can log onto your household account and update any information regarding your household if needed.
****If you do not know your log-in ID or password give us a call BEFORE the registration date and we are able to send you a reset password email.***

ways to register
- Online: If registering online & paying in full, you will be required to fill out Camp Forms at the time of online registration so please note you will need to allow for extra time during the registration process, about 15 min/child. Have all medical info, emergency contacts, and authorized pick-up info handy. Once you get through that process, you are done!
- Installments: If you are paying in installments, you MUST call the REC to sign up AND you will be required to complete your camp forms online BEFORE May 1. There will be a non-refundable $15 admin fee.
- Over the Phone: If you are planning to register over the phone paying (either in full or installments), you will be required to complete camp forms online BEFORE May 1.
- In Person: Come see us! As always, you may also come into the Rec to register for camp. We can register you and assist with setting up your online I.D so that you may complete forms at home!
ways to PAY
- Pay In Full: The best way to insure your child will enjoy summer camp this year is to pay in full at the time of registration. If this is a financial challenge you may also make installment payments.
- Installments: If you are paying in installments, you MUST call the REC to sign up AND you will be required to complete your camp forms online BEFORE May 1. There will be a non-refundable $15 admin fee.
- Pay by installments plan
- 25% due at registration, Note this amount includes a $25 non-refundable camper deposit per camper enrolled
- 50% by 4/4/2024
- 75% by 5/3/2024
- 100% by 6/7/2024
- * There is an additional $15 non-refundable admin fee charge per household if paying by installments
